Jessica Pawlicki, APRN
Jessica Pawlicki APRN, FNP-C is a board-certified Nurse Practitioner and credentialed through the American Association of Nurse Practitioners. She earned a Bachelor’s degree in Nursing from Chamberlain College of Nursing and later obtained a Master’s degree in Nursing as a Family Nurse Practitioner from Olivet Nazarene University. She is apart of a Functional Medicine program and an active member of the American Holistic Nurses Association, where she is learning how to better herself to focus on optimal functioning of the body and its organs, involving systems of holistic or alternative medicine. She holds a dual certification in Bioidentical Hormone Pellet Therapy and IV Nutrition Therapy.

Nichole Erhardt AGPCNP-BC
Nichole Erhardt APRN, AGPCNP-BC is a board-certified Nurse Practitioner and credentialed through the American Nurses Credentialing Center. She obtained her Bachelor of Science in Nursing from Purdue University and began her career in cardiology. While working in a hospital system, she grew frustrated that many medical providers overlooked the power of nutrition and preventative care. After dealing with her own health concerns that were corrected only with functional medicine, she was determined to become a functional medicine provider to help those around her. She completed her Master of Science in Nursing degree from Walden University and earned her board certification as an Adult-Geriatric Primary Care Nurse Practitioner. Her professional experience includes integrative physical medicine and she joined Well Rooted Functional Medicine in February 2022. She is a member of the Institute for Functional Medicine and holds certification in IV Nutrition Therapy. Her goal is to help her patients live the best quality life possible for as long as possible.
Michelle Lehn APRN, FNP-BC
Michelle Lehn is a board certified nurse Practitioner credentialed through the American Nurses Credentialing Center.
She graduated with a Bachelor’s of Science in Nursing degree from Northern Illinois University and went on to specialize in pediatrics for the following 15 years. In 2017, she chose to further her education and complete a Masters of Science in Nursing to become a Family Nurse Practitioner. The last two years she has worked in an urgent care setting treating acute ailments.
A personal health journey led her to find her true calling and passion in functional medicine. She is currently a member of The School of Applied Functional Medicine studying to further her knowledge in functional medicine.
Her goal is to educate and empower her patients to own their personal health journey in order to optimize their quality of life.
